The Cardiac Service provides advice, care and support to those living with Heart Disease in Somerset as well as to their carers and relatives. It also provides advice and information to health professionals caring for people living with heart disease. The aims of this service is to

enable people living with heart disease to achieve the best possible quality of life

provide and support evidence based therapies to those living with heart disease including exercise, education and medical therapy

offer emotional and psychosocial support to people living with heart disease and their families/carers

support and enable people to self manage their condition reducing their risks of a further cardiac event/ hospital admission

work with other care providers to improve the quality of care for people living with heart disease e.g stop smoking, weight reduction, end of life care
This service is provided by a team of Specialist Cardiology Nurses who deliver a Community Cardiac Rehabilitation Programme.
Community Cardiac Rehabilitation Service
The Cardiac Rehabilitation Service offers support and advice to patients following diagnosis of Coronary Heart Disease (CHD). This includes advice on physical activity, education on lifestyle change and support on psycho-social issues.

a telephone call and contact link for patients within 7 working days of referral

option of a home based facilitated programme

comprehensive assessment of individual progress

a six week supervised exercise and lifestyle programme

a link with primary and secondary care

a source of specialist advice for other professionals
Referral criteria
Patients are generally referred by the District General Hospitals following Myocardial Infarction, Angioplasty and Coronary Artery Bypass Graft.
