The Twilight Nursing Service and the Over Night Nursing Service is an extension of the District Nursing Service offered during the day. This service is for people who require nursing care, support or advice in their own home, a residential home or similar in the Out of Hours. It is for people who are housebound or who have some other valid reason that makes a home visit appropriate. Each request is considered individually.
Their work includes

direct skilled nursing care in a patient’s own home or in residential homes

assessing health needs, planning the care required, and involving other agencies/professionals as appropriate

supporting people whose problems would otherwise mean they had to be admitted to hospital

helping people with long-term, chronic conditions to live at home as independently as possible

post-operative care

wound care, pressure area care and leg ulcer management

catheter care
-
palliative and end of life care to enable people to die at home if they wish

health promotion and education to help people stay healthy and independent

drug administration via injection or central lines
The Twilight and Over Night Nursing Service consists of a Team Leader, Registered Nurses and Health Care Assistants.
