How to refer

All referrals must be made using the Integrated Therapy Service Referral Form.

Please note - Additional information forms

• If you are in a school and referring a child with speech, language and/or communication needs, you will need to attach a completed Communication Competencies in School form.

• If you are referring a child for Occupational Therapy assessment, you will need to attach a completed Additional Information to Support Occupational Therapy Referral form.

• If you are in a pre-school and referring a child with speech, language and/or communication needs, you will need to attach a completed Every Child a Talker Monitoring form.

Completing the relevant forms will provide the therapist(s) with valuable information for planning the assessment. You can download the forms by clicking on the MS Word icons.

Telephone Advice Line

The Integrated Therapy Service has introduced a new Telephone Advice Line.

Who is the Telephone Advice Line for?

The line is available for any professional or parent/carer who has concerns over a child or young person's development in the areas covered by Paediatric Speech and Language Therapy, Occupational Therapy or Physiotherapy.

Please note - the child or young person the call is about must be:

• aged 0 - 19 years
  
• a resident in Somerset or registered with a Somerset GP
  
• not already referred to the ITS

All enquiries about a child or young person who is already known to the ITS should be made to the appropriate area base. Details can be found on letters or reports from the ITS relating to the child or young person and are also available on the Contact Us page of this website.

What are the hours of the Telephone Advice Line?

09.00 to 12.00 in the morning - Monday, Wednesday, Thursday and Friday (not Bank Holidays)
