NHS Somerset is currently undertaking a procurement process for Telehealth products and services based on lots 3 and 4 of the Buying Solutions (Office of Government Commerce) national framework contract RM784. The overall purpose of the requirement is to provide home-based Telehealth monitoring for patients across Somerset with long term conditions, focusing initially on Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF) and Diabetes. This is part of an integrated response by NHS Somerset to the increasing number of individuals with chronic diseases and the need to identify innovative and effective ways to meet their clinical needs
This service will be seen as an enabler to the overall aim of introducing strategies to promote self care and self management, and to support primary care and community teams to effectively monitor patients at risk of exacerbations and hospital admission.
