The forms listed below should help staff access training across the Trust, both internally and externally.

Training Booking Forms

If you are booking for yourself after line managers approval you will need to complete the individual booking form.

If you are a line manager wanting to book more than one member of staff onto a course you will need to complete the group booking form.

If you are not a member of Somerset Primary Care Trust but want to access a course run by us you will need to use the form below.

Evaluations are welcome for all courses that we run and can be sent into us after the course has taken place.

Flexible Learning Form

This form must be filled in when staff access most types of flexible learning such as DVD's or e-learning, please see the flexible learning page for more information.
