What is Continuing Healthcare?
Continuing care means care provided over an extended period of time to a person aged 18 or over to meet physical or mental health needs which have arisen as the result of disability, accident or illness.
“NHS Continuing Healthcare” means a package of continuing care arranged and funded solely by the NHS.
NHS Somerset is responsible for arrangements for NHS funded Continuing Healthcare in Somerset
On 1 October 2007 the Department of Health launched the National framework for NHS Continuing Health Care. This guidance sets out the principles and process of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care. It concentrates mainly on the process for establishing eligibility for NHS Continuing Healthcare and principles of care planning and dispute resolution relevant to that process.
Continuing Healthcare will usually be applied for, on behalf of a patient, by a District Nurse, Community Psychiatric Nurse or Social Worker following an initial assessment.
